1. Date & Time: Include current date and time as you are filling it out. 

2. ⁠Pro Label (if required): Put the pro label on the indicated area. 

3. ⁠Shipper: Include shipper name, full address, contact, and pick up time and date. 

4. ⁠Consignee: Include consignee name, full address, contact, and delivery up time and date. 

5. ⁠Bill To: Include bill too name, full address, and contact email and number. 

6. ⁠Reference Number: Include the P.O. number and any other reference number pertaining to the shipment. 

7. ⁠Declared Value: Include the declared value of the shipment. 

8. ⁠Special Instructions: Include any special instructions and notes if any. 

Particular of the Goods

9. Pallets: Include pallet count of shipment. 

10. ⁠Piece(s): Include total pcs count of shipment. 

11. ⁠Weight: Include total weight of shipment and indicate measurement (kg or lbs).

12. ⁠Commodity: Inlcude commodity of shipment. 

Steps 13-20 may be repeated in below sections if shipment is mixed. 

13. Quantity: Include quantity of type (pallet, box/caton, crates, rolls, etc.). 

14. ⁠Type: Include type of packaging (pallet, box/caton, crates, rolls, etc.).

15. ⁠Dangerous Goods: Check box if shipment includes hazardous materials. 

16. ⁠Description: Inlcude a description of the goods. 

17. ⁠Weight: Include total weight of the goods described in the same line and indicate measurements (kg or lbs). 

18. ⁠Dimensions: Include dimensions of the “type” stated in the same line. 

19. ⁠NMFC: Include the NMFC number for the goods detailed in the same line. 

20. ⁠Class: Include the class number for the goods detailed in the same line. 

Notice of Claim

21. Notice of Claim: Ensure shipper, carrier/driver, and receiver print their name, sign, and date the bill of lading. Carrier/driver should also note the time in/out upon both the pick up delivery.
